HL7 Europe has published Base and Core FHIR Implementation Guides version 2.0.0
Brussels, 28 April 2026 — HL7 Europe has published version 2.0.0 of the HL7 Europe Base and Core HL7 FHIR Implementation Guides which are available for both HL7 FHIR R4 and R5. These guides provide a foundational framework to support consistent and interoperable implementations of HL7 FHIR across Europe. They define a common set of base profiles, extensions, and constraints designed to facilitate European initiatives, including the European Health Data Space (EHDS), and to support cross-border health data exchange.

Version 2.0.0 represents a significant step forward, incorporating new profiles, references to the latest EHDS-related logical models (Xt-EHR), improved model-to-profile mapping tables, terminology updates, and general publication cleanup.
The HL7 Europe Base and Core Implementation Guides are intended for a wide range of stakeholders, including national programs, healthcare providers, vendors, and research initiatives working on interoperable health data solutions.
By publishing both R4 and R5 versions, HL7 Europe supports the current implementation landscape while preparing the transition towards future HL7 FHIR releases.
The publication of these implementation guides follows the successful ballot and comment period of January 2026, which saw input from 230 individuals, representing 29 countries worldwide, including 23 European countries. This collaborative effort also includes representatives from Xt-EHR Joint Action and MyHealth@EU Solution Providers. The development of this Implementation Guide received support from the xShare and XpanDH projects.
